SMA Negeri 4 Blitar merupakan sebuah sekolah menengah atas di Kota Blitar, Jawa Timur, Indonesia. Sekolah ini mempunyai sebutan Forsma. Sama dengan SMA di Indonesia pada umumnya, masa pendidikan di SMA ini ditempuh dalam waktu tiga tahun pelajaran, mulai dari kelas X sampai kelas XII. Sekolah ini mulai beroperasi pada tahun ajaran 2011-2012 untuk mengakomodasi jumlah pelajar SMA di Kota Blitar yang semakin bertambah.[1]
