Health Samurai is an American health information technology company that develops software for exchanging clinical data using the Fast Healthcare Interoperability Resources (FHIR) standard. Its main products are Aidbox, a FHIR server platform, and Formbox, tooling for the FHIR Structured Data Capture (SDC) specification.[1][2]

History

Health Samurai was founded in 2004 as a healthcare software services team, and around 2014 the team shifted its focus to the FHIR standard, subsequently building the Aidbox FHIR server platform.[2] The company is legally organized as Health Samurai, Inc., with headquarters in San Pedro, California.[3] Related legal entities include Digital Samurai (United States) and Health Samurai DOO (Serbia).[3]

Aidbox

Aidbox is a FHIR server platform developed by Health Samurai.[4] In a 2025 peer-reviewed evaluation of the SQL on FHIR v2 specification published in npj Digital Medicine, Aidbox was used together with Pathling — a runner developed by CSIRO — as one of two independent implementations to validate the specification across a dataset of approximately 580 million FHIR resources; both runners produced identical results.[1]

Formbox

Formbox (originally released as Aidbox Forms and renamed in 2026) is a tool for building and running clinical questionnaires and forms based on the FHIR Structured Data Capture (SDC) implementation guide.[5] In 2025, HL7 International ran a Global AI Challenge to showcase standards-based innovation in healthcare AI.[6] Formbox received the Challenge's Clinical Data Quality and Outcomes Award.[7]

Industry participation

Health Samurai is an organizational member of HL7 International, the standards development organization responsible for the FHIR standard.[8]

In a 2025 vendor benchmark of the CMS Patient Access API published by the vendor Flexpa, Health Samurai was listed among thirty payer-side implementations reviewed for FHIR conformance.[9]

Academic partnerships

In 2025, Kathmandu University's Nepal Electronic Health Record Modernization (NEEM) programme signed a memorandum of understanding with Health Samurai for FHIR-based interoperability work.[10]
