Some studies show prolonged rest may interfere with body's natural inflammatory response and recommends some activity.

As when a tissue is injured, the body sends immune cells and chemical signals to begin the healing process. This process is enhanced by some movement which helps blood flow, brings in nutrients and stimulates the tissue to rebuild. Prolonged rest slows this process and if an injured area is completely immobilised for too long, blood flow can not only be reduced but immune cells may also reach the tissue more slowly and the mechanical signals that helps tissues repair themselves are absent. As a result, without movement - tissue repair is slower and muscles and joints can become stiff and recovery can take longer. The inflammation itself isn't bad when it's actually essential for the healing process as it stimulates release of growth factors like IGF-1 that rebuilds the tissue. For these reasons, early movement is recommended per latest evidence.[1]

Major caveat is that movement is permitted as long as it clearly doesn't aggravate pain. More specifically pain limited gentle movement to help speed recovery.

obviously - early movement is beneficial only to the extent it doesn't worsen the injury. Overdoing it or moving through sharp pain can actually delay healing or cause further damage.

Follows evolution of views and older protocol. Mention research had shown that prolonged rest can delay recovery, leading to the POLICE protocol that introduced gentle pain-free mobility exercises. Because of research showing no effectiveness of icing for healing, Peace & Love followed later as a modern update to POLICE, removing icing completely from the guidelines. Note was introduced in peer reviewed sports medicine British Journal in 2019.

Promising for AFC for two reasons; it's published in a peer reviewed British Journal and it follows a body of evidence. The other acronyms don't have high quality evidence to back them up, and is on borrowed time.

In summary - by 2020s , the PEACE & LOVE - PEACE (Protection, Elevation, Avoid anti-inflammatory drugs) and LOVE (load, optimism, vascularization, and exercise) - is the introduced update to traditional older injury‑management acronyms R.I.C.E., PRICE, and POLICE. It prioritizes gradual rehabilitation, patient education on factors like appropriate level of inflammation and well-being, with ice no longer being taught as essential to recovery.

Timeline of evolution;

Acute soft-tissue injury management has evolved over time following systematic reviews and studies especially since 2010.

For decades - the traditional protocols focused on ice to reduce pain and swelling, however that was not evidence-based.

In 1978, it was Dr. Gabe Mirkin who coined the term R.I.C.E. (Rest, Ice, Compression, Elevation) and recommended without evidence that limiting inflammation promotes healing. Somehow it spread widely. It did however limit inflammation and pain, but at cost of delayed healing.

Around 1992 - protocol expanses further to PRICE (Protection, Rest, Ice, Compression, Elevation) with the P to emphasize protecting the injured tissue from further damage.

By 2006 - the POLICE guideline was introduced - (Protection, Optimal Loading, Ice, Compression, Elevation); which uses the updated principle of early, controlled movement but only after reflecting a summary of evidence proving that prolonged rest can delay recovery. It still recommended icing.

More research on icing was conducted. Systematic review consistently show among many studies showing insufficient evidence to support icing on faster healing. Ie, a 2014 study shown ice did not improve healing outcomes but instead delayed recovery following eccentric exercise-induced muscle damage, resulting in a rebound of muscle hemoglobin concentration compared with the controls.

Finally in 2019, the most recent recommendation was introduced in 2019 aka the PEACE & LOVE (Protection, Elevation, Avoid anti-inflammatory drugs)

It is significant as it no longer regards RICE or specifically (icing) as essential to recovery.

As of 2026, expert consensus and systematic review increasingly leans towards PEACE & LOVE (Protection, Elevation, Avoid anti-inflammatory drugs) protocol rather than RICE due to one having actual abundance of evidence and the other not having any from the start.

Important notes; however as of today, people seem slower to abandon long time advice. RICE is still recommended by more than a few reputable bodies however it is clear there is now a growing trend among science community recommending against icing following injury from other reputable bodies.

PEACE & LOVE may have a lame name -the original people who coined the acronym were Canadians physiotherapists and appears to be traced to the British Journal of Sports Medicine in 2019 by Blaise Dubois and Jean‑François Esculier.

Mainstream media and general public medical sites such as WEBMD and Healthline haven't abandoned RICE but peer reviewed journals like British Journal now recognize PEACE & LOVE. Also it should be noted most reputable journals still recognise RICE and have not officially abandoned it completely so it's still a matter of debate between the two. Both sides should then be covered in history section rather than claiming outright that PEACE & LOVE is the only standard today and RICE has been abandoned when that is clearly not the case at the moment.
